I acknowledge by signing this document that NINJEW TATTOO has given me full opportunity to ask all questions I might have regarding obtaining a tattoo. All of my questions have been answered to my complete satisfaction. My signature also acknowledges that I have been specifically advised about the facts and matters set forth below and my agreement to the following:

1. I do not have hemophilia, nor do I have diabetes, epilepsy, hepatitis, HIV, AIDS, or any other communicable disease.
2. I am not under the influence of alcohol and/or any drugs.
3. I acknowledge that it is not reasonably possible for NINJEW TATTOO to determine whether I might have an allergic reaction to the pigments or process used in my tattoo, and I agree to accept that such a reaction is possible.
4. I acknowledge that infection is always possible as the result of obtaining a tattoo, particularly in the event that I do not take proper care of the tattoo, and I agree to follow all instructions concerning the healing process.
5. I agree any touch-up work required due to my own negligence will be done at my own expense.
6. I realize that variation of colour and design may exist between the tattoo I have selected and how it appears on my skin when compared to someone else.
7. I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove the tattoo.
8. To my knowledge, I do not have any physical, mental, or medical impairment or disability that may affect my wellbeing as a direct or indirect result of my decision to have any tattoo related work done at this time.
9. I acknowledge that I have truthfully represented to NINJEW TATTOO that I am over the age of 18 years, or have the full consent of my parent/guardian and over the age of 16.
10. I acknowledge that obtaining my tattoo is my choice alone, and I consent to the application of the tattoo and to any action or conduct reasonably necessary to preform the tattoo procedure.
